Our administrative staff is ready to help you with any questions about scheduling, financial policy and insurance, to make that part of the process as simple as possible.
Your First Visit
Your initial appointment will consist of a thorough examination, photos and necessary x-rays for proper diagnosis, depending on your needs. Study casts may also be taken. Occasionally, treatment can be done the same day. However, a complex medical history or treatment plan will require a consultation. This will involve a second appointment to discuss your individual needs with the doctor.
Please assist us by providing the following information at the time of your initial appointment:
- Your completed new patient registration forms, these can be found on our website or can also be mailed if needed.
- Any current X-rays
- A list of medications you are presently taking.
- If you have dental insurance, bring your insurance card.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the initial appointment. Please alert the office if you have a medical condition that may be of concern prior to your appointment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or you require medication prior to dental treatment (i.e antibiotics, for pre-med.)
If your previous dentist has taken recent x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our office.

We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at (810) 364-9060. Many times, a simple telephone call will clear any misunderstandings.
For your convenience we accept cash, check, Visa & MasterCard. We also offer no interest payment plans through CareCredit and LendingPoint.
The parent/guardian accompanying the minor is responsible for any co-payments at the time of service.
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

It is important that we have diagnostic xrays for your new patient appointment so that we may diagnose any necessary treatment. If you have had any xrays taken within the last 5 years, please fill out our release form so your xrays can be transfered to our office.
